survey_instruction (type: esriFieldTypeString, alias: Please fill out this form based on current stock, expected need, and items on order. For each category there are four numbers that should be supplied to help the DOC understand need and coordinate supplies across the system.
In Stock: Current supplies in the district or location that you are reporting on.
Total Needed: This number is used to charectorize supply levels in your location for the DOC as: Good, Low, or Critical.
On-Order: This is the number that your location has been able to secure via an order from a ventor but have yet to physically receive the supplies.
Request from DOC: This is the number of supplies in a catagory that have been requested from the DOC.
