intro_note (type: esriFieldTypeString, alias: The Ministry of Health is undertaking a climate change risk assessment for 34 hospital sites located across Aotearoa New Zealand.
You are invited to complete this survey by (Date) to identify relevant climate change risks to these sites., SQL Type: sqlTypeOther, length: 255, nullable: true, editable: true)
background_note (type: esriFieldTypeString, alias: Climate change will have significant implications for the Ministry of Health.
The graphic below summarises likely climate change implications for Aotearoa New Zealand. This can be used as a guide when identifying risks to the Ministry of Health sites.
More information is available on the NIWA webpage https://niwa.co.nz/our-science/climate/information-and-resources/clivar/scenarios, SQL Type: sqlTypeOther, length: 255, nullable: true, editable: true)
instructions_note (type: esriFieldTypeString, alias: The survey is set out so that you can identify and describe the climate risks to the sites, buildings, facilities and critical infrastructure that serves it.
There are three broad hazard groupings:
Flooding and extreme rainfall
Coastal Hazards
Temperature increases and drought
Fill in your contact details and select which site you work at.
For each hazard listed, add an element at risk and a description. Press the (+) button to add additional elements. To review the hazards you have listed, click on the numbers beside the (+) button.
Any additional climate hazards you wish to identify can be added at the end.
Example 1:
Hazard: Flooding and extreme rainfall
Element at risk: Underground carpark
Description: The carpark floods during heavy rainfall
Example 2:
Hazard: Temperature increase and drought
Element at risk: Energy supply required for air conditioning units
Description: Increased usage of energy during high temperatures means the hospital will overwhelm the aircon units currently in use.

hazard_casading_explainer (type: esriFieldTypeString, alias: Note: Climate change risks can be both direct and indirect. A direct risk occurs as a result of a climate hazard (e.g. increased temperature/ flooding) impacting on something of value e.g. building, facility, infrastructure etc. An indirect risk occurs often as a result of knock-on/ cascading effects from a direct risk. For example, a direct risk would be sea level rise causing a building to flood. An indirect risk would be the impact on the ability for patients to access the building, which in turn could prevent access to medical care., SQL Type: sqlTypeOther, length: 255, nullable: true, editable: true)
